Our entire community's well-being is at stake. As a not-for-profit health care provider, we offer health and wellness programs at no or low cost as part of our mission.
Outreach Services works to evaluate and respond to these critical needs in our community. These services include the operation of our community health centers, medical education, subsidized care, early detection and prevention programs, screenings, health fairs and more.
Community Health
We care for all who need it. We provide everything from wellness education and preventive screenings out in the community to comprehensive primary care at our community health centers. In addition to the traditional services, our clinics offer medication assistance programs for patients who qualify. For more information call 574.252.0359
Mobile Medical Unit
Our Mobile Medical Unit, a 40-foot-long clinic on wheels that enables SJRMC to meet the needs of individuals who may not have transportation or access to medical offices or clinics. With the MMU, we can also deliver corporate wellness to the doors of local organizations, providing health education and screenings on-site. For more information call 574.252.0361
Prenatal Care Coordination
Prenatal care coordination is a service developed to improve outcomes of pregnancy and reduce infant mortality rates. This service includes assessment, education, referrals and support. This outreach and home visiting program targets pregnant women who may be at risk due to medical or psychosocial factors. School Health Initiatives
In agreement with Penn-Harris-Madison and School City of Mishawaka, SJRMC provides a nursing paraprofessional in each school who works in conjunction with either a consulting nurse or an on-site registered nurse. School health aides and nurses have over 180,000 school health visits annually. Aides and nurses assist with wellness services and health fairs. SJRMC also underwrites the School Health Ministry program of the Catholic Schools Office for the schools in the South Bend area of the Diocese of Fort Wayne-South Bend. Senior Services
We offer many support services to seniors in our community. Our SHIP (State Health Insurance Assistance Program) counselors provide assistance to those who need help with Medicare or other related health insurances. Tobacco Initiatives
As the lead agency for Tobacco-Free St. Joseph County, Saint Joseph Regional Medical Center works to eliminate the tobacco industry's influence through advocacy, social alteration and policy change. Smoking Cessation Classes are available free of charge. For more information call 574.252.0362.
