School City of Mishawaka Employee Clinic

The School City of Mishawaka Employee Clinic (Clinic) provides access to convenient and high-quality primary care and wellness services. It is owned by School City of Mishawaka (SCM) and managed and staffed by Saint Joseph Regional Medical Center (SJRMC) as a member of the Saint Joseph Physician Network.

Eligibility

All School City of Mishawaka employees and retirees along with their dependent children and spouses, who are enrolled in the School City of Mishawaka Health Plan, are eligible to receive health services at the SCM Clinic. While the SCM Clinic is managed by SJRMC, employees who use the services can continue to use the hospital of their choice.

Cost

There are no co-pays or deductibles to use the clinic services, including medications dispensed by the SCM Clinic. Patients may also have labs drawn through the clinic (processed through the South Bend Medical Foundation) at no cost to the patient.

Services

The SCM Clinic strives to facilitate a collaborative partnership with local providers for the benefit of our mutual patients. With patient consent, SCM Clinic will send copies of all pertinent medical records to other providers. We make every effort to avoid duplicating care, particulary lab and radiology testing.

Outside regular Clinic hours, patients who wish to speak to a nurse about non-emergency medical questions can call 855.388.3234 to be directed to the TeamHealth® Nurse Triage Call Center. In event of emergency call 9-1-1. If patients wish to ask a question regarding appointment scheduling or their records, they should leave a message on the Clinic answering machine or use the website link for SCM Clinic Question Link.

Appointments

Appointments with the physician or nurse practitioner are normally scheduled for 20-40 minutes depending on if it is an established or new patient. Appointment scheduling is available online at www.mishawaka.k12.in.us. Click Staff Links and then click NEW Employee Portal. The portal is operated by No More Clipboard and will guide you to identify the desired type of appointment. This is the most efficient process to schedule appointments for a provider, nurse lab draw and/or medication refill pick-up. Follow the instructions to set up an account and request an appointment. Patients may also call the Clinic directly at 574.855.1514.

Late Arrivals and No Shows

Please arrive five minutes early to your appointment and check in at the front desk. New patients should plan on arriving 15 minutes prior to thier first scheduled appointment.  Please call if you are going to be late. Patients who repeatedly schedule but do not keep or cancel their appointments may lose privileges.
