Transitional Care Center
The Transitional Care Center is a skilled nursing facility located on the fifth floor of Saint Joseph Regional Medical Center— Mishawaka. Twenty beds are licensed by the state of Indiana and certified by Medicare to provide patients with the skilled nursing care sometimes needed after surgery, injury or illness. The unit is ideal for those who no longer need the resources of acute hospital care yet may need some medical attention during their rehabilitation or recovery.
What are the advantages of staying in a
hospital-based
unit?
The Transitional Care Center is able to provide quality skilled nursing care and still offer immediate access to all of Saint Joseph Regional Medical Center’s services. Laboratory tests, rehabilitation services, x-ray and other diagnostic and therapeutic services are within easy reach. Also, without the inconvenience of transfer to another facility the patient is free to concentrate more fully on his or her recovery.
What is the average length of stay?
The center is not intended to be a long-term residential facility, but is designed for those who need a short stay prior to returning home. Depending on their needs patients may stay anywhere from a few days to a few weeks.
What healthcare services are available through the Center?
24-hour medical coverage is provided by our qualified and compassionate nursing staff. Additional medical support is available as needed, including physical, occupational and speech therapists, and orthopaedic and diabetes educators.
Is emergency medical care available?
The Transitional Care Center is staffed with an experienced team of healthcare professionals trained to offer preventive and emergency medical care. Because the center is located within the hospital, rapid access to acute care or emergency services is available when needed. For your peace of mind, a physician is on call 24 hours a day within the hospital for any emergency that may arise.
What insurance is accepted?
Medicare Part A and Part B coverage (or other applicable Federal Social Security programs) are strongly recommended. Blue Cross and Blue Shield or other similar medical and hospitalization insurance plans are also welcome.
Visitation
Visitation is open and friends or relatives can visit at their convenience.
