Carolynn Horn

PharmD, BCPS, BCACP - Preceptor, Ambulatory Care Pharmacist 

Dr. Horn graduated from the University of Cincinnati in 2010, followed by the SJHS Ambulatory Care PGY1 in 2011. During the course of her residency, she created a business plan which she subsequently submitted to Owensboro Medical Health System, landing her a pharmacist position in her hometown. She worked in the Health System’s charitable clinic before transitioning to the hospital’s outpatient pharmacy. After taking time off to enjoy her new role as adoptive mother, she began working and continues to work at SJHS remotely three days a week. Her professional interests include ambulatory care, specifically transitions of care, and is blessed that she is able to work in this field at SJHS.
